In order to attend the intensive all boarding and commuting students MUST fill out the forms that apply. Please pay close attention. We highly suggest completing any forms that apply as quickly as possible.

Health Assessment Form

Students in grades 3-12 MUST submit the "Health Assessment Form" or one of the same equivalence that has been filled out within 36 months of August 16, 2025. If you have graduated High School you do NOT need to submit this form.

Physical Exam

If you have graduated High School you MUST submit this form or a letter from a Doctor stating you are in good health to engage in this type of activity. Students in grades 3-12 do NOT need to submit this form.

Authorization of Medication

Authorization for the Administration of Medication

Students with medication MUST have this form filled out by a parent and prescriber. ALL medications must be brought in original prescription bottles and will be given to the on site nurse for the duration of the intensive. That includes over the counter medicine as well.

Individual Plan of Care

If an individual has special healthcare needs or disabilities such as allergies, an order for an EpiPen, asthma, special dietary needs, chronic illness, hearing or visual impairments MUST fill out this form. If special care and plan of action is already noted in the "Health Assessment Form" then you do not need to fill this out.
