Forms
In order to attend the intensive all students MUST fill out the forms that apply to them.
Please return forms back to us no later than July 25th.

IMPORTANT: Please ensure that each form is uploaded as a single file. For example, when submitting your Health Assessment Form, only one file can be uploaded, so all pages of the form must be combined and included in that file.

Health Assessment +
Physical Examination
All attendees, regardless of age, must submit the "Health Assessment Form" or one of the same equivalence that has been filled out within 36 months of August 14, 2026.
Medication Authorization +
Individual Plan of Care Form
Students with medication MUST have this form filled out by a parent and prescriber. ALL medications must be brought in original prescription bottles and will be given to the on site nurse for the duration of the intensive. That includes over the counter medicine as well.
If an individual has special healthcare needs or disabilities such as allergies, an order for an EpiPen, asthma, special dietary needs, chronic illness, hearing or visual impairments, you MUST fill out this form. If special care and plan of action is already noted in the "Health Assessment Form" then you do not need to fill this out.
Parent Nurse Authorization Form
All attendees must fill out this form.
Dorm Policy Form
All attendees who have signed up for the Dorm Experience must fill out this form.​
